New Patients

We recently switched to Tebra EMR. The fastest way to get setup as a new patient is to email our office requesting an appointment. In this email provide your first and last name, DOB, and mobile phone number (these are required to create you as a patient in the EMR). Then in-take forms can be securly sent to you to fill in demographics information, history, insurance, etc.

  • New Patients: please arrive 15 minutes prior to your appointment time to fill out new patient forms (via tablet kiosk) or fill out ahead of time (via in-take forms). Please bring your Driver's License, Medical Insurance Cards, and a credit card for copayments (we accept Discover, Visa, Mastercard, Amex, or cash).
  • If you call our office after hours, please leave a clear message. It is important you spell your last name and tell us your phone number with area code two times slowly, so we may have the correct phone number to call you back. Phone messages are checked often after hours and your messages will be returned and addressed promptly.
